A REAL series of unfortunate events..
I had a nightmare last night..Dreamt about weird stuff, like organ harvesting, people collapsing in theatre, ghosts, and somehow also got pinched on the butt by a ghost. So eerie..i woke up in cold sweat, and i dare not even move an inch or open my eyes..When i finally did, my HP showed that it was only 3:55am..Luckily i managed to doze back to sleep, and this time without any dreams..
Naturally, i woke up with eye bags, and feeling like i've never slept properly the whole night, and the nightmare still fresh in my head..I went to work feeling damn suay, and tired..Like its a bad omen..And true enough, i reached work, and there is a P1 neuro case rushing in from ED..That woman took MC again, leaving only kak zub and me who are morning..So kak had to take over reception 2, where it was chaos as there were alot of 1st cases to check, and all the patients came in all at the same time..So poor me had to run between OT 2 and OT 17 which was running an appendicectomy...See, this is where i get my daily exercise..haha..
We are always very superstitious about the people we work with in EOT, as we know a few who always bring business to us..So, we can expect how the day will turn out..And today's anesthetist on call is tsc, well known for bad cases during her call..sigh.. Her first case for today was a saucerisation of a sebaceous cyst on the back. I wouldn't think anything would happen to the pt..i mean, its just a cyst loh..The op was uneventful..But after extubation, the pt wasn't breathing well, sats were dropping to 80+, 70+ %..and somehow the pt was not responsive, but his eyes were open..by this time, tsc was shouting orders, and scoldng dr ckk for transferring the pt to the trolley so fast..we had to transfer the pt back to the op table, re-intubated the pt, set an arterial line..Then, pt was having frothy whitish foam coming out of the ETT, so we suctioned the ETT, and gosh, i was sucking out alot of liquid out of the ETT..Then Dr tsc said that pt has pulmonary oedema..Its like pt is drowning from all the fluid in the lungs.. The CXR showed the left lower part of the lungs not visible anymore..So poor pt had a urinary cath inserted, transferred to ICU intubated..What a dramatic first case right..And its my first time seeing the pulmonary oedema too.
This is not the end of the series of unfortunate events..OT 18's anesthetic machine checked showed leakage in the system early in the morning..Fredrick and i checked like 10 times before we changed the breathing circuit, and true enough, it is the breathing circuit that is the problem..For the whole time that i am here, the chances of getting a spoilt breathing circuit is like close to zero, because most of the time, it is either the soda lime that is not fitted well, or something that is not closed properly..So today i got the spoilt breathing circuit, i must say is really very very suay!!
I was so busy the whole day that me and my junior did not go for break..I sent her for break only at 1.30pm..And i could only go around 2, when i am supposed to go off at 2.30pm..sighz..And by 2.30pm, i heard tat the EOT board is already full..Every space on the board is filled with cases..I really pity the afternoon staff..and even the night..for sure it will drag till next morning..
I went for driving lesson after work, and i end up having to drive with leg cramps..Especially during parking..I was on the verge of dozing on the wheel already..
What a day!! I'm so glad i'm home safely now..oh, i dread tmr..i have 3 thyroidectomy & 1 tonsillectomy..
