Last night, or rather this morning....Was horrible + terrible + sadness........
We started off with a bang, and ended off with a big bang as well...
We just arrived at work, and we have 4 theatres running, not warmed up yet, and then Dr Chong came in saying there is a polytrauma case rushing up from ED. She is what we call a jumper..When she came in, blood was dripping everywhere, from almost all of her orifice, her clothes were soaked in blood, the bedsheets were soaked in blood..We were racing against the blood loss, transfusing blood..All her veins are shutting down, plus the edema, it was impossible to get an iv cannula..Her heart stopped beating occasionally..Boluses of adrenaline after another...We used a total of almost 50 ampoules of adrenaline! The radiologist team came up and did some kind of angio, not very sure what they were doing...Then, the ortho team started X-fixing all her broken limbs...And we sent her down to the CT scan room..After which she was brought to the ICU.
After the whole drama, i saw my clothes and shoe was also covered with her blood, the floor was covered in so much of blood and mess, all my equipment had to be throughly cleaned..Yet, we had to do everything fast, coz we are an EOT, and we had other cases waiting to be done, like appendicectomy, which was held up because of the polytrauma. We finally settled down, and decided to close the theatres at around 5.30am and get some rest...Just as we were sitting down, updating the boards, Dr chong called us and said that the same jumper had to come back in for a laparotomy, as they found from the scans internal bleeding..
So it was a round 2 for us..I felt like it was a boxing match..But this time round, she was in a much more critical condition, her blood pressure was constantly low, so, yeah we just kept on diluting adrenaline..CPR kept on going..There was a conflict as to whether should the operation continue or not, and whether was she going to make it...1 doctor said, "Proceed with the operation, since we know where the bleeding is." Another said, "There is no point..She will collapse once the operation starts, and there would be even more blood loss." The final decision was being made by the main surgeon, and senior consultant that the operation will not be done..And she was sent back to the ICU, which i think she won't be coming out from..
Conclusion: Is it really worth it?? We used up about 100 ampoules of adrenaline, about 30 packs of blood products (Yes, it was blood type O..See how much we use for 1 patient! Go donate blood, people!!) , yet at the end of the day she is still going to die? She jumped, she wanted to die, yet she didn't die, and was being brought to the hospital where medical staff are fighting to save her life..Well, this is called ethics..We are medical personnels, we are required to save all in need of saving, be it whether they chose to die or not..At the end of the day, it is our job. Sighz, what a night!
(p/s: i have sworn not to eat baos when i am on shift. We have this belief that bao is a jinx item, because bao in chinese, directly translates to wrapping up, which to us means wrapping up dead bodies..Well, Naina and i both ate bao before going to work last night, So, was it really the bao that's why we got the polytrauma case to come in twice? Its hard to say..)
